The Transitional Care for Medically Complex Children (TCMCC) program is a multi-sector partnership program between Safehaven and Holland Bloorview Kids Rehabilitation Hospital in Toronto, Ontario. This innovative new model provides integrated transitional care from hospital to community for medically complex children and youth leaving lengthy hospital stays.
